Achilles Tendon Ruptures
Do you prefer early rehab or an immobilized cast? What are your thoughts on the pros and cons of each?

My rehab protocol for acute Achilles tendon repairs is:
1. Frontslab below knee (BK) NWB for 2/52
2. If wound ok at 2/52= BK equinus cast allowing WB as tolerated for the next 4/52
3. At 6/52 postop = Aircast boot with 2 heel wedges - FWB / remove one wedge every 2/52 - starting passive ROM exercises - progressing to active ROM and proprioception exercises
4. Usually the pt is able to walk FWB without boot at 10-12/52

there is a lot of evidence that early mobilization improves the outcome, operatively or non-opertivly treated (read twaddle 2007).
