Talus necrosis

I am not a doctor.
I am searching for answers and/or opinions.
My husband has a hole in his talus. It was found recently with X-ray and Resonance imaging. It has roughly 19 mm of diameter. He has pain when walking.
We have already obtained orthopaedic opinion and they are suggesting a bone graft harvested from iliac to avoid fracture.
My husband was an intense sport person on his youth. He has now 43 years old. He is tall, 1,85 cm height and weights roughly 95 kg. We live in Lisbon, Portugal.
This necrosis might be due to a sequence of sprains on his ankle that caused a decrease of blood circulation in that area.
Is surgery the only way to solve it? Can we do something else that could improve vascularization.
